## The short answer

**Sermorelin prompts your pituitary to release your own growth hormone, while HGH supplies the hormone directly.** HGH is approved for specific conditions, and the DEA describes off-label anti-aging prescribing of it as illegal. Neither is approved as an anti-aging treatment. Sermorelin is what a provider can consider through Astra, compounded and not FDA-approved. We did not identify a trial that compares them for adult wellness goals, so it cannot be said that one is proven safer or better.

Both can raise IGF-1, a hormone the liver makes in response to growth hormone, and that blood marker is commonly used to track either treatment.

## Why people compare them

The design argument for sermorelin is that it works through the body's own signalling. Because the pituitary still responds to feedback, [release stays intermittent](https://pubmed.ncbi.nlm.nih.gov/18046908/) rather than constant. That difference is real, but a design argument is not the same as outcome evidence. No large trial we found shows that sermorelin gives the same benefits as HGH with fewer harms.

For HGH, a [systematic review in healthy older adults](https://pubmed.ncbi.nlm.nih.gov/17227934/) found small changes in body composition, increased adverse events, and concluded it cannot be recommended as an anti-aging therapy. Another [review on athletic performance](https://pubmed.ncbi.nlm.nih.gov/18347346/) found claims of enhanced performance unsupported.

The closest direct comparison we found is in children, not adults. In a [1989 study of 7 children with GH deficiency](https://pubmed.ncbi.nlm.nih.gov/2537727/), GHRH therapy did not improve growth in 5 of them, and growth improved in all 7 after switching to recombinant GH. It was a very small study, and a [later, larger study](https://pubmed.ncbi.nlm.nih.gov/8772599/) reported better growth with GHRH on a different schedule.

## Common questions

### Is sermorelin as effective as HGH?
We did not find a trial that answers this for adult wellness goals. HGH raises growth hormone directly, while sermorelin depends on your pituitary's response.

### Can I take sermorelin and HGH at the same time?
We found no evidence supporting this. Do not combine them unless a specialist directs it. The [Geref label](https://assets.hpra.ie/products/Human/16600/LicenseSPC_PA0285-006-001_13072006155719.pdf) says those already on GH should stop it before its diagnostic test.

## Safety at a glance

Reported side effects include flushing, injection site pain or irritation and mild headache. Sermorelin is not recommended in pregnancy or breastfeeding, active cancer or certain pituitary conditions, and long-term safety in adults has not been established.
