Is sermorelin safe? Side effects, risks and unknowns
What regulatory labeling and small studies report, who should avoid sermorelin, and what remains unknown.
.md Read this page as Markdown (for AI assistants)The short answer
Small studies report mostly mild, short-lived side effects, such as flushing and injection site pain, and a provider review helps screen out people who should avoid it. Long-term safety in adults has not been established in large trials, and compounded sermorelin is not FDA-approved or reviewed by the FDA for safety, effectiveness or quality. That is why who prescribes it, and who prepares it, matters.
How Astra approaches safety
A review before anything is prescribed
A licensed US provider reviews your health history, and can decide sermorelin is not right for you.
A licensed pharmacy
A state-licensed US compounding pharmacy prepares it on a prescription for you, not a research-labelled vial.
Someone to ask
You can message your care team if you have questions or side effects after you start.

| Effect | Source |
|---|---|
| Facial heat, facial flushing and injection site pain, usually gone within minutes | Geref label, Ireland. This product was a diagnostic test, so it reflects single doses |
| Injection site redness or irritation, flushing, mild headache | Astra's sermorelin page |
| Temporary rise in blood lipids, which resolved by the end of the study | Khorram, 1997, 19 older adults, a closely related GHRH analog |
| Generally well tolerated, with no change in fasting glucose or excess IGF-1 | Thorner, 1996, 110 children with GH deficiency |
These sources do not give reliable rates for adults using sermorelin long term, so the list should not be read as complete.
Who should not take it
- Pregnancy or breastfeeding. The Geref label lists this as a contraindication.
- Known allergy to sermorelin or its ingredients (Geref label).
- Active cancer or certain pituitary conditions. Astra's sermorelin page says it is not recommended for these.
- Extra caution: the label advises particular care in people with diabetes or epilepsy for the diagnostic test. Tell your provider if either applies.
What providers may check
IGF-1 is a hormone made mainly in the liver in response to growth hormone, and it is the usual blood marker for GH activity. Our sermorelin guide explains why it is part of a careful evaluation. Blood sugar matters too if you have diabetes or take glucose-lowering medication. The Geref label also notes that untreated hypothyroidism or anti-thyroid medication may affect the response to the drug, so mention any thyroid condition.
What is still unknown
The adult trials we reviewed were small and lasted 16 weeks or less. They cannot show what happens over years. Sermorelin works by raising GH and IGF-1. For comparison, the label for tesamorelin, a related FDA-approved drug, warns that the effects of prolonged IGF-1 elevation are unknown, and warns about fluid retention and glucose intolerance. Those warnings apply to tesamorelin, and it is not known how far they apply to sermorelin.
GH itself has a clearer record: a systematic review in healthy older adults found small body composition changes with increased adverse events, and said it could not be recommended as an anti-aging therapy.
Compounded medications bring their own considerations. FDA explains how compounded drugs differ from approved ones.
Common questions
Is sermorelin allowed in sports?
No, not for tested athletes. The World Anti-Doping Agency's 2025 Prohibited List names sermorelin among substances prohibited at all times.
How long can you safely take sermorelin?
We found no large trial that establishes a safe duration in adults. Your provider can review whether to continue, based on how you respond and your lab results.
Does sermorelin affect your heart?
The studies we reviewed were not designed to answer this, so it remains unknown. Long-term cardiovascular safety has not been established even for tesamorelin, which has larger trials. Tell your provider about any heart condition.
Is sermorelin safer than HGH?
They have not been directly compared in a way that settles this. See sermorelin vs HGH for what each record shows.
Do side effects go away?
The Geref label says flushing and injection site pain usually disappear within minutes. Report anything persistent or severe to your provider, and seek urgent care for signs of an allergic reaction.
